I made a 2x3.5 using the Avery 5371 business card template in Google Docs: front shows “BC > Blue > Red/Gold (SST) > Green > Lavender > Gray,” back lists additives (sodium citrate, clot activator/SST, heparin, EDTA, fluoride/oxalate). If you print on synthetic paper and 5‑mil laminate, it survives coffee spills and 2 a.m. draws; just double-check your site’s CLSI/GP41 policy in case serum tube details differ.
